The doctor details are not optional
A fitness certificate is a medical declaration, not an institution letter. The useful format therefore puts the doctor name, qualification, registration number, examination date, signature, and seal in the document body.
The National Medical Commission maintains the Indian Medical Register, and the NMC Code of Medical Ethics requires registered practitioners to maintain records of medical certificates issued. That is why the registration number and date fields are prominent.
