Government Medical College and Super Facility Hospital, Chakrapanpur, was founded in the year 2006 in Azamgarh, Uttar Pradesh. National Medical Commission (NMC) recognised this as an affiliate college which provides numerous medicine courses for the aspiring healthcare specialists. It has a 98-strong faculty to accommodate the student body for varying specialisations in medicine at the same time.

The college is well-equipped to support students' academic and personal growth. Accommodation facilities for the boys and girls are provided for living comfortably by the college, with sports facilities, and an auditorium for co-curricular activities and all other events. An on-campus medical facility is also established to ensure health care among the students.

The IT infra and the cafeteria further contribute to a good campus. Government Medical College and Super Facility Hospital, Chakrapanpur, offers a total of four full-time courses. The MBBS programme forms the basis of medical education and has an enrolment of 100 seats that is approved.

For postgraduates, the college provides MD in Community Medicine with 8 seats, MD in Pharmacology with 4 seats, and MS in Anatomy with 4 seats. Admissions will be based on the National Level Entrance Exams into Government Medical College and Super Facility Hospital, Chakrapanpur. For the MBBS program, admissions will be done through NEET. Postgraduate students will have to clear National Eligibility cum Entrance Test for Post Graduate ( NEET PG ) to be eligible for admission into the MD and MS programmes.
